Waiver, Release, and Assumption of Risk Form

This form is an important legal document. It explains the assumption of any and all risks in deciding to follow the advice or insight, or use of products, from Fabienne Louis, medical intuitive, intuitive healer, content writer, qigong & sound practitioner, and artist, who is the sole proprietor of Fabienne Louis, LLC or any representative of that Company. It is critical that you read and understand it completely. After you have done so, please sign this form. Thank you.

Waiver, Informed Consent, and Covenant Not to Sue

I understand that Intuitive Healing/Medical Intuitive service/Qigong & Sound work is a relationship I have with my practitioner designed to facilitate the creation/development of my best life, and what I express how my practitioner can assist. I understand that intuitive healing is a comprehensive process that may involve all areas of my life and could include work, finances, health, relationships, education, activities and that deciding what to do in these realms is my responsibility. 

I understand that the information I give to Fabienne Louis/Fabienne Louis LLC is confidential unless I state otherwise, in writing. However, I understand that if I report abuse, neglect, or threaten to harm myself or someone else, necessary actions will be taken and my confidentiality agreement will not limit this capacity. 

I hereby acknowledge I have volunteered to participate in a paid or unpaid session, class, teleseminar or program with Fabienne Louis to include, but also may not be limited to, any and all services provided, such as energy healing ,remote sessions, intuitive guidance, classes, product offerings and workshop attendance. In consideration of Fabienne Louis’s agreement to instruct, assist, advise, or train me, I do here and forever release and discharge and hereby hold harmless the Company and its respective agents, heirs, assigns, contractors, and employees from any and all claims, demands, damages, rights of action or causes of action, present or future, arising out of or connected with my participation in a session or purchase or any program WITHOUT LIMITATION, WHICH MAY OCCUR AS A RESULT OF following advice tendered and released or training rendered or use of facilities during a session or event.

I recognize that in no way does Fabienne Louis provide legal, medical, financial, or therapeutic advice and it is my responsibility to secure such advisement.

I acknowledge and agree that I assume the risks associated with any and all activities, offerings, products, services, classes and/or programs in which I participate, or purchase.

I acknowledge and agree that no warranties or representations have been made to me regarding the results I will achieve from any insight, guidance or program. I understand that results are individual and may vary.

NON-DISCLOSURE and INTELLECTUAL PROPERTY: I understand that the methodology, recording of the sessions, coaching techniques and strategies used, as well as assignments, physical & digital products, documents or emails are proprietary and I understand that and agree that they may not be used for any other purposes other than my coaching appointments without written consent from Fabienne Louis. I understand that information is held as confidential to fulfill my intuitive healing obligations and as required by law.

I ACKNOWLEDGE THAT I HAVE THOROUGHLY READ THIS WAIVER AND RELEASE AND FULLY UNDERSTAND THAT IT IS A RELEASE OF LIABILITY. BY SIGNING THIS DOCUMENT, I AM WAIVING ANY RIGHT I OR MY SUCCESSORS MIGHT HAVE TO BRING A LEGAL ACTION OR ASSERT A CLAIM AGAINST Fabienne Louis/Fabienne Louis LLC FOR NEGLIGENCE OR THAT OF ITS EMPLOYEES, AGENTS, OR CONTRACTORS.

I understand that I am responsible for the fees if I cancel in less than 24 hours. Missed sessions will not be made up.

I am choosing to participate in intuitive healing services with Fabienne Louis/Fabienne Louis LLC. Any suggested Qigong Practices, Sound Healing, Meditation, or Reading Material is just a suggestion. It is my choice to perform or read any suggested material and I do them at my own risk.
